Health Forms

Your Health Committee has set up a database to store information on the health of our Field Spaniels.We have updated the original Health Forms to include further test results that we now cover on our Health Testing Days. The forms have also been combined into the one form for ease of completion. The form can now be downloaded in a choice of formats. However, please remember to update us with any further information on the health of your dog when necessary.

The forms are dealt with in the strictest of confidence. Further copies are available for download below :

Below we have the form to be filled in and submitted with your blood sample to the Animal Health Trust when having a Thyroid Test carried by your Vet. The testing will be done and results given in the same way as those carried out by your Vet's labatory but the advantage will be that the results will be used and collated with others to get a Thyroid level for Field Spaniels. The series of 3 tests with the AHT and Cambridge Specialist Labs are combined and offered at a discounted rate of £65,00 plus VAT
